About Me
My name is Sarah Kalvoda, and I am a Primary Health Care Nurse Practitioner. I have lived in Uxbridge since childhood, and grew up on an Equestrian Center just outside of town. I have 13 years of experience in many different areas in Nursing including; Medical/Surgical, Public Health, and Emergency Medicine. As a Nurse Practitioner, I have practiced in Stroke Prevention, and Family Medicine.
Throughout my career it has always been an aspiration to open up my own practice, and now my dream has become a reality.
I have a passion for providing quality health care services to individuals that goes far beyond expectations. Healthcare is extremely challenging to navigate for many people, and I have personally experienced barriers during the past several years in accessing care on different levels. I always strive to assist individuals to obtain their best possible health. I have an extremely respectful, warm, and compassionate approach to care and I look forward to providing exceptional care to the community where I grew up.

Visit Types
Acute Episodic
$75 (20 minutes)
To assess an area of concern using a comprehensive more in-depth approach. During this visit, the Nurse Practitioner will thoroughly assess, diagnose, and treat minor illnesses. Minor illnesses might include but are not limited to; ear infections, upper respiratory tract infections, skin infections, urinary tract infections, sexually transmitted infections, ect. Prescriptions will be included during this visit (if medically necessary). Clinician notes can be provided if necessary for work/school purposes (additional fees may apply depending on time and work involved).
Comprehensive Visit
$200 (60 minutes)
To assess multiple areas of concern using a comprehensive more in-depth approach. The Nurse Practitioner will consider the multiple factors that may be contributing to the areas of concern. Chronic diseases can be evaluated and optimized using an evidence-based approach. During this visit the client will be actively involved in selected treatment plan that is tailored to individual needs, which might include traditional or alternative approaches. During this appointment clients will be given the full opportunity to express mental or physical health concerns that they have not previously been able to fully express.
Drivers Medical
$140 (30 minutes)
Clients will have the opportunity to have a driver’s medical completed in a timely manner without any barriers.
Procedure
$50 (15 minutes)
Clients will be able to have ear flush, plantar wart removal, skin tag treatment/removal.
Prescription Renewals
$50 (15 minutes)
Clients will have the opportunity to have non-opioid/benzodiazepine prescriptions renewed and filled after the appropriate assessment is made.
Drivers Medical
$140 (30 minutes)
Clients will have the opportunity to have a driver’s medical completed in a timely manner without any barriers.

$999 + HST
- Same day/next day access to in-person clinic visits 7 days/week (with exception of statutory holidays)

- Unlimited prescription renewals
- Referral management

- Maximum 25 visits/year, any additional visit will be subject to pay-per use fee of $65 (reduced price)
Couples Annual Membership:
$799 + HST
- Same day/next day access to in-person clinic visits 7 days/week (with exception of statutory holidays)
- Unrushed appointments
- Unlimited prescription renewals
- Referral management
